Apple Creek Health & Rehab is located in quiet, Centerton, Arkansas. We are the newest all-private room long term care/skilled nursing facility in NWA, and we are Medicare and Medicaid certified. Our staff includes Licensed Physical, Occupational, and Speech Therapists, Licensed Practical Nurses, Registered Nurses and Certified Nursing Assistants.
